New Patient Intake

Getting started with your diabetes supplies shouldn’t be stressful. At Diabetic Supplies Inc., we make it easy for new and returning patients to begin or continue their orders for Continuous Glucose Monitors (CGMs), insulin pumps, and pump supplies. Our Intake Team handles the paperwork, insurance checks, and communication with your provider so you don’t have to.

Here’s a clear, step-by-step look at how intake orders work and what to expect from start to finish.

Step 1: Patient Request or Referral

1. Patient Request

You contact us directly by phone, email, or fax to request a CGM, insulin pump, or supplies.

2. Provider Referral

Your doctor, diabetes educator, or clinic sends us a prescription or referral form. Providers using Parachute Health can send prescriptions securely online.

3. Insurance or Manufacturer Referral

Sometimes your insurance or device manufacturer (Dexcom, Medtronic, Tandem, Omnipod, etc.) sends us your information to help fulfill your order.

Once we receive your information, our Intake Department creates your secure patient profile and begins processing your request.

Step 2: Insurance & Prescription Verification

Our Insurance Verification Team:

Check your insurance benefits to confirm coverage
Determines whether prior authorization is required
Contact your provider for chart notes, labs, and prescriptions
Ensures we have everything needed to move forward

If additional information is needed, we’ll reach out to you or your provider right away.

Step 3: Prior Authorization (If Required)

If your insurance plan requires prior authorization (PA), our team:

Prepares all required forms and documentation
Works directly with your provider to collect signatures and clinical records
Submits the PA request to your insurance
Provides updates once approval is received or if more details are needed

Approval times typically take 7-21 business days, depending on your insurer.

Step 4: Order Confirmation & Patient Consent

Once coverage is approved, we will contact you to:

Review your order
Confirm shipping information
Discuss your estimated copay or out-of-pocket cost
Obtain your consent before shipping

Nothing ships until you give your approval.

Step 5: Shipping & Delivery

After authorization and patient approval, your order ships directly from our distribution center.

You’ll receive a tracking number by email
Most deliveries arrive within 1-3 business days
Recurring supply orders are set up automatically (every 30 or 90 days, depending on insurance)

Our goal is fast, reliable delivery every time.

Step 6: Reorder & Follow-Up

Our Reorder Team makes sure you never run out of supplies. We will:

Notify you when it’s time for your next shipment
Verify insurance and contact details
Update prescriptions or documentation when required

You can also request refills anytime by phone, email, or fax.

Documents We Typically Need

Prescription for your CGM, insulin pump, or pump supplies
Provider clinical notes from the last 6 months
Recent A1C or glucose log (if required by insurance)
Front and back of your insurance card(s)
Signed patient consent form

We coordinate directly with your provider, especially those using Parachute, so you don’t have to gather everything yourself.

Processing Times

Most intake requests are completed within:

1-2 business days for existing patients with valid prescriptions and insurance
1-15 business days for new patients or those requiring prior authorization
