At Diabetic Supplies, Inc. (“DSI”), every patient has the right to safe, high-quality, respectful, and accessible service. We provide diabetes-related durable medical equipment—including Continuous Glucose Monitoring (CGM) systems, insulin pump devices, and related supplies—and we are committed to treating all clients with dignity, fairness, and professionalism.
Your Rights as a DSI Client
1. Right to Dignity, Respect, and Non-Discrimination
- Be treated with courtesy, dignity, and respect at all times
- Receive services without discrimination based on age, race, gender, disability, medical condition, national origin, sexual orientation, or insurance status
- Have your personal, cultural, and spiritual values respected
2. Right to High-Quality Service
- Receive medically necessary equipment in accordance with CMS, Medicaid, and insurance guidelines
- Expect safe, properly maintained, and clinically appropriate equipment
- Receive accurate information about the equipment ordered, including benefits, risks, alternatives, and limitations
- Expect continuity of service and timely follow-up
3. Right to Privacy and Confidentiality
- Have your medical and personal information protected under HIPAA
- Request restrictions on how your information is used
- Access your records upon request
- Receive a copy of DSI’s Notice of Privacy Practices
4. Right to Informed Participation
- Receive clear explanations about your medical equipment, insurance coverage, responsibilities, and supplies
- Be involved in decisions relating to your care
- Refuse equipment or services (within insurance rules)
- Be informed about any changes in equipment, suppliers, or service arrangements
5. Right to Transparent Financial Information
- Receive explanations of coverage, deductibles, copays, and out-of-pocket costs
- Request itemized billing statements
- Be informed of Medicare, Medicaid, and private insurance appeal rights
6. Right to Access and Accommodation
- Accessible services and communication formats
- ADA-compliant reasonable accommodations
- Assistance understanding forms, paperwork, and instructions
- Alternative formats upon request
7. Right to Safe and Proper Equipment
- Receive safe, compliant equipment
- Receive training, instructions, and troubleshooting help
- Timely replacement of malfunctioning equipment
- Safe delivery and support
8. Right to Voice Concerns and File Complaints
- Express concerns without retaliation

- Select any qualified DME supplier
- Transfer service at any time
- Receive assistance transferring records or prescriptions Client Responsibilities
- Provide accurate medical and insurance information
- Notify DSI of any changes
- Use equipment safely and as instructed
- Report equipment issues promptly
- Pay required deductibles, copays, or balances
- Treat DSI staff respectfully
